District/ Regional Sport Trial Forms

​District (Bayside) Trials:

To nominate to attend a District (Bayside) Trial, students must follow the below steps:

1. Trials will be advertised on Daily Student Notices.
2. Go to the HPE Staff Room in U Block and REGISTER their details on the Registration Sheet
3. Collect the following forms from HPE Staff Room:
1. Trial information Sheet
2. Standard District Form
3. Project Consent Form 
4. Nomination form/Player history (Only if requested by Convenor)
4.  Return completed forms to HPE Staff Room by the due date or ASAP for lodgement.

Please Note: The school will organise the Principal Consent form for students using the Registration Sheet information. Therefore it is imperative that students write the details on the Registration Sheet.

Students are notified at the Trial of the team selection and will receive Regional (Met East) forms and information at that time.

Regional (Metropolitan East) Trials (Individual nominations):

To submit an individual nomination for a Regional (Met East) Trial, (Where there is no District Trial held prior) students must follow the below steps:

1. Trials will be advertised on Daily Student Notices.
2. Go to the HPE Staff Room in U Block and REGISTER their details on the Registration Sheet
3. Collect the following forms from HPE Staff Room:
1. Trial information Sheet and Nomination form
2. Standard District Form
3. Project Consent Form 
   
4. Return completed forms to HPE Staff Room by the due date or ASAP for lodgement. Alternatively, you can scan and email forms to tmcke82@eq.edu.au
5. WAIT… Wait for confirmation/invitation email to attend Trial from Convenor/CDSHS – then PAY Regional Trial Levy ($22) online via the Met East Sport website:  https://rssshop.education.qld.gov.au/met-east
6. If payment is made after due date, proof in the form of a receipt number may be required to take to the trial.
